Chart Import (Apple Health Records)
Chart Import brings a person's clinical chart — medications, allergies, immunizations, lab results, conditions, and vital signs — into CareHaven from a file you download at your patient portal, or, for the phone's own owner, straight from Apple Health. Available
On this page
Open it from Main Menu (bottom left) → Medical → "From an imported chart" → Imported records → Import records, then choose Import from a file or Import from Apple Health. The Import records button is there from the start — even before you've imported anything for this person, the empty Imported records screen offers the same button.
Imported items land as read-only evidence from your chart — held with their source and date, shown alongside your own records but never mixed into them. Nothing in your chart changes, and nothing leaves your device. You decide, item by item, what actually becomes one of your own CareHaven records.
What comes in, and what doesn't
Chart Import reads six kinds of clinical data: medications, allergies, immunizations, lab results, conditions, and vital signs. Everything else in a chart file or Apple Health record — visit notes, procedures, insurance/coverage details — is left out.
Every imported item stays on your device only, by design — it doesn't sync even if you turn sync on for the rest of your data, isn't shared with a co-caregiver, and doesn't show up in Search. Apple Health import only ever reads the profile you've marked as the phone's owner — it never touches anyone else's profile.
Bring a chart in from a file
- At your patient portal, download your records — look for a "Download my record" option. It usually comes as an XML file (a C-CDA / Continuity of Care document) or a ZIP bundle; CareHaven reads either one, plus a FHIR export if your portal offers that instead. A PDF After-Visit Summary isn't supported yet — if that's the only option, check your portal for the XML or ZIP download instead.
- Open Main Menu (bottom left) → Medical → "From an imported chart".
- Tap Imported records.
- Tap Import records, then Import from a file.
- Choose the file you downloaded. CareHaven reads it on your device and lists what it found.
This works even before you've imported anything for this person — the empty Imported records screen has the same Import records button.
If the file turns out to be for a different person than the profile you're in, CareHaven stops and tells you — a chart can't land on the wrong profile by accident.
Bring a chart in from Apple Health (phone owner only)
Apple Health import only works for the profile marked as the phone's owner. If you haven't set that yet:
- Tap the profile name pill (top center) to open the profile switcher.
- Tap the active profile row again to open it, then Edit Profile.
- Scroll to the Apple Health section and turn on "This profile is me – sync sleep, cycle, and vitals with Apple Health."
- Tap Save.
Once that's set:
- In Imported records, tap Import records, then Import from Apple Health.
- The first time, CareHaven shows a short note about what it reads and that it stays on your device. Tap Continue.
- Approve the Health permission when iOS asks. CareHaven reads your Health Records and lists what it found.
This only works if your iPhone's own Health app already has Health Records set up and connected to a hospital or clinic that supports it — that connection is something Apple's Health app handles directly, not CareHaven, and it depends on your provider offering it. Import from Apple Health only appears on the profile marked as the phone's owner, and only on a real iPhone — it isn't available in the Simulator or on an iPad without Health. Everyone else still has file import.
Look through imported records
Under Medical → Imported records, items are grouped by type and show:
- a From your chart tag, so it always reads as evidence from your chart, not something CareHaven is claiming;
- the status as your chart recorded it (for example, "Marked active by the source") — CareHaven never restates it as its own judgment;
- an item marked entered-in-error by the source, shown struck through rather than hidden;
- Seen in an earlier import on an item that showed up in a past import but not the latest one — CareHaven never says something "stopped," only what the chart showed;
- Looks different when the same item comes back changed from an earlier import (a different dose, say). CareHaven shows both values side by side with their dates and never tells you which is right — that's a question for your prescriber.
Add to my records
Open an imported medication, allergy, immunization, or condition and tap Add to my records to bring it into your own records. Nothing is added automatically — you review each item first.
- Medications and allergies: choose to link the chart's item to one you already track, or add it as new. If the chart lists a different dose than you have, CareHaven flags it as something to check with your prescriber — it never changes your saved dose for you. Linking an allergy also adds it to your mealtime food-safety warnings.
- Conditions: added through CareHaven's condition list; confirming one can turn on its extra guidance, but only when you choose to.
- Lab results and vital signs stay as read-only evidence in Imported records — they don't have an "Add to my records" step yet.
Anything you've added can be removed again from its detail screen. Removing an item you linked to a record you already had never deletes your own record — only the link.
Share a Care Summary
You can put together a provider-ready Care Summary PDF from what you keep:
- Open the Caregiver area.
- Tap Care Summary.
- Choose what to include — sensitive categories start off, so turn one on only if you want it in this PDF.
- Check the included/hidden count, then tap Create.
A Care Summary only ever includes records you added to your records or wrote yourself — never un-reviewed imported evidence. Every page is marked patient/caregiver-authored — not an official medical record, so it's never mistaken for a clinician's own chart.
Delete imported chart history
To clear the imported evidence for a person while keeping everything you've added to your records:
- Open Settings → Backup & Files → Manage imported data, or open Imported records and look for the same option there.
- Choose the person, tap Delete imported chart history, and confirm.
This removes the imported evidence only — anything you added to your own records stays exactly as it was.
Open a chart file from Files, Mail, or AirDrop
You don't have to start inside CareHaven to bring a chart in. If you have a chart export file (the XML/C-CDA or FHIR JSON your portal gives you) sitting in the Files app, attached to an email, or sent by AirDrop, just open it there and choose CareHaven — the file opens straight into CareHaven instead of making you find it again from Imported records.
Before anything imports, CareHaven asks which person the chart belongs to. Confirm the person, then review what CareHaven found, the same as any other import.
Known limitations
- PDF chart files aren't supported yet. If your patient portal only offers a PDF After-Visit Summary, look for the XML (C-CDA) or ZIP download instead.
Privacy, in short
- Imported records and Apple Health data stay on your device — never synced, never shared with a co-caregiver.
- Apple Health import only ever reads the profile marked as the phone's owner.
- CareHaven only reads your chart — it never writes anything back to your patient portal or to Apple Health.
- A Care Summary you create is yours to share; CareHaven never sends it anywhere on its own.
See Medical for everything else CareHaven tracks for a doctor, a diagnosis, or a medication.
